WAIVER AND RELEASE OF LIABILITY

I understand that football is a physical sport involving risks that may include:

Falls and collisions
Sprains and strains
Fractures
Concussions
Heat exhaustion
Injuries during training and matches
Travel-related accidents

I voluntarily allow participation in Yakoma Soccer Academy activities and accept these risks.

from liability arising from participation in academy activities except where prohibited by law.

MEDICAL AUTHORIZATION

In the event of illness or injury, I authorize academy representatives to obtain emergency medical treatment for the participant.

I understand that all medical expenses remain the responsibility of the player and/or parent/guardian.

PROPERTY DAMAGE AGREEMENT

I accept responsibility for any damage caused by the participant to academy property, facilities, vehicles, accommodation, equipment, or third-party property.
Any costs incurred may be charged to the participant or parent/guardian.
