Personal Property

Yakoma Sports Academy (General Santos City, Pangasinan, and Manila, Philippines) shall not be held responsible for the loss, theft, or damage of any personal belongings, equipment, valuables, money, electronic devices, or other personal property brought to or used during any Academy program, football training session, tournament, event, camp, or activity.

Participants and their families are responsible for safeguarding their personal belongings at all times.

Assumption of Risk and Release of Liability

Participation in football and related sporting activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, or, in rare circumstances, death.

By enrolling a participant in any Yakoma Sports Academy program, parents, guardians, and participants acknowledge and accept these risks and voluntarily assume full responsibility for participation.

To the fullest extent permitted by law, Yakoma Sports Academy, its directors, officers, employees, coaches, staff, volunteers, sponsors, partners, venue providers, and affiliated organizations shall not be liable for any injury, illness, death, loss, damage, cost, or expense arising from or related to participation in Academy activities, except where such liability cannot legally be excluded.

Parents and guardians agree to indemnify and hold harmless Yakoma Sports Academy and its representatives against any claims, actions, proceedings, liabilities, costs, or expenses arising from the participant’s involvement in Academy programs.

Emergency Medical Authorisation

In the event of an accident, injury, illness, or medical emergency, and where a parent or guardian cannot be contacted within a reasonable time, or where immediate action is required, Yakoma Sports Academy and its authorized staff are permitted to:

Arrange and authorize emergency medical treatment;
Arrange and authorize emergency dental treatment;
Administer basic first aid;
Seek medical advice from qualified healthcare professionals;
Arrange emergency transportation to an appropriate medical facility when necessary.

The Academy will make reasonable efforts to contact parents or guardians as soon as possible.

Medical Expenses

Parents and guardians acknowledge and accept full responsibility for all medical, dental, hospital, transportation, medication, rehabilitation, and related expenses incurred as a result of illness, injury, accident, or emergency treatment involving the participant.

Yakoma Sports Academy does not provide medical insurance coverage unless otherwise stated in writing.

Medical Information Disclosure

Parents and guardians are required to disclose all relevant medical information, health conditions, allergies, disabilities, injuries, medications, or special care requirements on the registration form before participation.

This information is essential to help ensure the safety and well-being of participants.

All medical information will be treated as confidential and will only be shared with authorized personnel, healthcare providers, or emergency responders when necessary for the participant’s care and safety.

Failure to disclose relevant medical information may affect the participant’s ability to safely participate in Academy activities.

International Travel and Overseas Programs

For any international tournaments, football tours, exchange programs, training camps, scouting events, or educational experiences conducted outside the Philippines, participants and their families are solely responsible for obtaining and maintaining comprehensive travel, medical, accident, and personal insurance coverage for the entire duration of travel.

Such insurance should include, but not be limited to:

Medical and hospital expenses;
Emergency evacuation;
Personal accident coverage;
Travel cancellation or interruption;
Loss of baggage and personal belongings;
Repatriation expenses.

Yakoma Sports Academy accepts no responsibility for medical costs, travel-related expenses, insurance claims, losses, delays, cancellations, or other costs incurred during international travel programs.

Parent and Guardian Declaration

By signing the registration form, parents and guardians confirm that they have read, understood, and accepted these Liability, Medical Consent, and Insurance Terms and Conditions and agree to be bound by them.
